Washington Middle School

The Montana State Board of Health has a vaccine requirement for all seventh grade students. All incoming seventh grade students must show proof of having received one dose of Tdap (tetanus, diphtheria, and pertussis) vaccine before attendance is allowed. Parents may complete an immunization exemption  form for medical or religious reasons. These forms may be requested from the middle school.
